The following programs and services are offered through Northwest Dane Senior Services (NWDSS), Inc.

Mobile Meal Program
Mobile Meals are nutritious mid day meals delivered to your home by volunteers. These meals are available Monday through Friday with an additional meal on Friday for Saturday to home bound seniors. (Donation)

Congregate Nutrition Site
Midday meals are offered Monday through Friday, at the Connection Room at NWDSS offices in Black Earth. This nutrition program also provides education and information, and serves over 4,000 meals to seniors every year. Reservations are required by calling NWDSS at 767-3757 please contact at least one day in advance for your reservation.

Supportive Home Care
The Supportive Home Care Program provides services to individuals who require assistance to remain living in their own homes. These services may include, light housekeeping, meal preparation, laundry, light personal care, lawn care and in-home respite. (Fee)

Rural Respite
This social activity program is designed specifically for older adults who need more structure in their life. Contact NWDSS office for day and time availability for the program.

Case Management Services
As the only senior service provider in northwestern Dane county, the Case Manager advocates for the client to ensure they receive the assistance they require to remain living at home and in their community. The Case Manager may offer short term or ongoing assistance by; helping sort through complicated paperwork (insurance, financial or medical), arranging transportation, or making appropriate recommendations and referrals to other community providers. The Case Manager offers necessary assistance that enhances the lives of seniors, preventng premature placement in a facility. (Donation)

Foot Care Clinic
Pedicure services and blood pressure screening are offered by a nurse monthly on the third Tuesday of the month. Please contact the NWDSS office for an appointment and costs.

RSVP Driver Escort
Transportation service for seniors providing both local rides and trips to Madison. Call NWDSS to schedule your driver through RSVP. A donation is requested to RSVP of Dane County per ride received.

Group Transportation
There is a Bus/Van available for area seniors. Contact NWDSS for scheduling of grocery shopping, nutrition site attendance, medical appointments, and Activity Center trips. Please contact the NWDSS office for reservations and costs.

Activity Center
This program offers seniors a safe place to gather for fellowship, games, potluck luncheons, trips, wellelderly programs, and parties are just a few of the exciting events held at the Activity Center. The volunteer activity committee assists with planning the calendar. The list of various activities are available in our monthly newsletter.

Caregiver's Journey Group
The purpose of the Caregiver's Journey group is to learn new information, share ideas and support one another. The group is open to anyone who is a caregiver, family, or friend of an older person. Transportation and respite are available. The Caregiver's Journey group meets the second Tuesday of the month in Mount Horeb.
